North Bay Lodge is a 24-hour supervised residential home that provides living services to individuals who have a diagnosis of psychiatric illness or intellectual disability. It is also for individuals who have a related condition which results in impairment of adaptive behaviour similar to that of persons with intellectual disability, e.g. acquired brain injury.

Applicants admitted to North Bay Lodge should have the following characteristics or skills:

  1. Able to participate in programmed activities (i.e. vocational workshops and recreational activities, e.g. yoga, body exercises).
  2. Capable of basic self-help skills (i.e. dressing, feeding, bathing self, etc.) with ad hoc supervision.
  3. Demonstrates no behaviours that pose serious danger to self, others, or property.
  4. Ambulatory: able to self-arise from bed at a reasonable rate of speed (in event of emergency, i.e. fire).
  5. The individual does not require routine attendance during night-time sleeping hours.
  6. The individual must be capable of following directions (under emergency conditions).

Section A: Criteria for Admission for Independent Living

  • The individual is between 18 and 40 years of age.
  • Capacity for independent living, i.e. able to function dependently — especially with activities for daily living, e.g. dressing, bathing, eating, and all other aspects of personal hygiene.
  • Basic understanding of commands and at least minimal verbal communication.
  • Self-direction.
  • Economic self-sufficiency (via Grantor/responsible person).
  • The individual exhibits moderate deficits in adaptive behaviour.
  • The individual is overall healthy and free from communicable diseases.
  • The individual exhibits no extreme behavioural problems.
  • The mental condition is likely to continue indefinitely.
  • Updated medical reports, not older than 3 months, stating:
    (i) diagnosis & prognosis,
    (ii) current treatment (medicines prescribed) must be submitted.
  • Assessment report from the immediate last mental facility where the applicant was resident.
  • Financial stability: It is preferable if the resident is a beneficiary of a Trust with sufficient assets and cash flow to provide for their needs should the Grantor decease. Grantors will be required to have at least two signatories as Surety to the Life Rights Occupancy Agreement.
  • Admission is subject to a 3-month probation period from either party, which may be extended depending on circumstances and evaluation from North Bay Lodge management or Council.

Section B: Level of Care

Level-of-care determinations are based on variables affecting the developmental needs of each individual’s (i) adaptive behaviour and (ii) health status – physical impairment.

  • Criteria for Low Care escalation:
    Resident requires assistance with specific daily activities, i.e. bathing, dressing, and grooming, and is otherwise not under specific monitoring by carers.
  • Criteria for Moderate Care escalation:
    Resident requires assistance and guidance with most of their daily activities, e.g. bathing, dressing, shaving, eating. Resident receives routine monitoring by North Bay Lodge carers.
  • Criteria for High Care escalation:
    Resident requires 24-hour care, full-time supervision, monitoring, and assistance with all their daily living needs.

Insurance: in terms of the resident’s furniture and personal effects etc. in their rooms please note that these items are not insured within the North Bay Lodge insurance policy.

A minimum of two calendar month's notice is required should the resident wish to leave.

Our annual fee increases come into effect on 01 April.

The probationary period for new residents is 3 months. Permanent residence will be granted only once all the required information and conditions have been met to the satisfaction of management.

North Bay Lodge reserves the right to reconsider any permanent residence should future requests for Grantor participation not be addressed to our satisfaction, this would include issues like annual financial information, residential fees, resident’s medication, housekeeping e.g., toiletries, clothing, and the like.

Service Agreement:

  1. I, the duly authorized Debtor (Primary Grantor) do hereby acknowledge I am truly and lawfully indebted to NORTH BAY LODGE for the monthly fee, as amended from time to time, for the resident placed in NORTH BAY LODGE's care.
  2. The debtor undertakes to pay to NORTH BAY LODGE the current monthly residential fee on the 1st of each month, monthly in advance.
  3. In the event of the debtor failing to meet the regular payment, NORTH BAY LODGE reserves the right to discharge the resident and will, where possible, facilitate the transfer of the resident to another facility.
  4. If as a consequence of any default by the Debtor, NORTH BAY LODGE finds it necessary to consult with Attorneys or to incur any legal costs in connection with or relating to the enforcement of the Creditors rights in terms of or arising from this agreement of debt, the Debtor undertakes to pay the Creditor, on demand, the full amount of any such legal cost.
  5. The Debtor acknowledges that the fee can be reviewed and escalated on an annual or need basis.

Indemnity Form:

I, the undersigned, having granted permission for the Resident to reside at NORTH BAY LODGE do, by my signature hereto, indemnify and hold harmless NORTH BAY LODGE, the Trust, all staff, employees, Volunteers and helpers of the said organization against any loss, damage or injury which he/she may cause or suffer arising out of his/her presence or attendance at NORTH BAY LODGE.

This indemnity shall also apply in respect of loss or damage arising out of injury or death to him/her:-

a. During transportation
b. During any visits and excursions from NORTH BAY LODGE
c. During sports, games, exercise, horse riding or any other activity
d. Arising from his/her residence at NORTH BAY LODGE

Medication Indemnity:

This indemnity shall apply in respect to medication given to residents by NORTH BAY LODGE on behalf of the Primary Grantor (person responsible for paying applicant's fees). (It is the responsibility of the Primary Grantorr to ensure that all medication is supplied timeously and in sufficient quantities for the resident, as and when required.)
(It is the responsibility of the Primary Grantor to ensure that the resident attends the clinic / doctor timeously every month.)

NORTH BAY LODGE will require the following:-

a. A copy of the residents current script must be given to NORTH BAY LODGE
b. Any generics supplied must be accompanied by a note as to which medication it replaces
c. Any and all changes to medication must be given by the Primary Grantor in writing with a copy of the new script
d. All medication must be signed out by the Primary Grantor when taking the resident from the farm and signed back in upon returning. It is strictly prohibited that medication be in the possession of a resident.

The Primary Grantor indemnifies NORTH BAY LODGE against

a. Any occurrence resulting from a shortage of medication
b. Any occurrence resulting from the resident, despite NORTH BAY LODGE’s best efforts, failing to take his/her medication.
c. Any adverse effects from the medication being taken or any combination of medication being taken.
